Generic Health Plans:

Asthma Care Plan To be used with Asthma diagnosis. Separate Medication Form REQUIRED if a student will have an inhaler at school (kept with the student and/or in the health office).
Seizure Care Plan To be used with any seizure history.
Allergy Anaphylaxis Care Plan To be used for any allergy with a potential for anaphylaxis. Separate Medication Form REQUIRED for rescue Epinephrine kept at school.
Food Allergy Care Plan To be used for any food allergy that does NOT have any potential for anaphylaxis.
Diabetes Care Plan To be used with a diabetes diagnosis for diabetic management at school. REQUIRES a DMMP (Diabetic Medical Management Plan) signed by the student’s medical provider.
Cardiac Care Plan To be used with any cardiac diagnosis.

*If your child needs a more individual health plan based on their diagnosis, please contact the District Nurse assigned to your child’s campus. Please also be prepared to provide necessary medical documentation including a signed letter from a medical provider if necessary.
