Open Enrollment Information for Medical, Dental, and Vision 
 
Open Enrollment takes place in the fall of each year.  Please note that only employees wishing to change their current medical, dental, and/or vision coverage or opt-out status need to complete any forms.  
 
 
MEDICAL - Open enrollment with CalPERS for medical coverage effective January 1 takes place from mid September through mid October. Only those wishing to make a change for their medical coverage need to participate during this window by completing an HBD-12 form to add, delete, or change coverage. 
Certificated and Unrepresented staff that currently are taking medical coverage and wish to decline coverage during open enrollment and also qualify for cash-in-lieu (Full-time certificated and unrepresented employees only) will need to provide proof of medical coverage to receive funds and complete the Cash-in-lieu form. Classified staff do not have a cash in lieu program.
 
DENTAL/VISION - Open enrollment with CVT for dental/vision coverage effective October 1 runs from September 1 until the 3rd Friday of September.  Only those wishing to make a change for their dental/vision dependent coverage need to participate during this window by logging in at www.cvtrust.com website to add, delete, or change dependent coverage for dental/vision insurance.
